The overall structure of a document is as follows.
Title: one line description of what was done
Abstract: very short description of what was node

Appendices, if any (extra details if needed)

The sections of the document are as follows.
Introduction: short description of what was done, including brief conclusions, and contents of the document.
Background: background of the problem not in the following sections
Identification: Identification of the problem
Definition: definitions needed
Design: design of the solution
Implementation: important parts of the implementation (other parts can be in the appendices)
Evaluation of the design and implementation
Conclusions: conclusions and take-aways from what was done
Summary : short review of what was done, including brief conclusions.
